Possibly repetitive but another Ennio earworm which once again is a perfect obsessive pitch for the movie it depicts, complete with childs choir (or what sounds like it!)
"...The son of a wealthy industrialist has his psychosomatic paralysis treated by his psychiatrist aunt. What starts as innocent therapy and care soon develops into a manipulative, erotic and psychological battle..."

It is only technically not a western, but it's music is very much not like his westerns I know, so I feel good recommending Duck, You Sucker!, which is lovely, and at times comedic. It's as underconsidered as that wonderful film is.

Thank you again for the recommendations! On just the first four pages alone there are more than 50 scores listed over and above what I already own. I'm making notes, researching based on your recs, and listening.

Surprised nobody has metioned Canone Inverso, a lovely score, the highlight of which is the Finale Di Un "Concerto Romantico Interotto", a sumptuous 4 minute pastiche mini violin concerto. Listen to that and you can only imagine the disappointment when at a concert some years ago, Morricone did his viola concerto (perhaps with a pre-recorded viola part?) which turned out to be very much avant garde in style. However, Canone Inverso is terrific score, just a shame it never got turned into a full concert work (pastiche be damned!).
