These are in films too, but i just saw an episode of a TV show called Manifest that ha about 5 things wrong inside of five minutes. It had a nightclub on fire that had several things wrong with it: the nightclub didn't look like a nightclub, it looked like it was set-designed by people who had never seen one; the nightclub had about 30 people in it, the number multiplied into 3 times the amount when they were running out of the building after it went on fire; the fire was smoky but everyone was breathing fine for the most part; a security door was locked, but two men shouldered it down; everyone stood by in an orderly fashion and there wasn't a crush; best of all was the exterior shot - they wanted a bit of busyness in the shot, but if you watched it, seven firefighters ran into the burning building, none with hoses, none with breathing apparatus, but five had axes, one had a torch and one had a long stick, then two cops run into the building holding nothing, then some of the axe-wielding firemen run out. It was comical.

When police, FBI, etc turn up outside a suspect's apartment, house, farmhouse, whatever, they always slam their car doors when they get out...and then move stealthily. They never leave them open or shut them silently, like any sensible person would as to not alert the suspect. I hear my neighbour's car doors slamming 5 doors away so any suspect on the run would have plenty time to climb out the rear window and escape. This door-slamming blunder is in so many films and cop shows.

This door-slamming blunder is in so many films and cop shows. Not to mention all of the screeching tires of the cars arriving at the location! If they really wanted to catch a suspect completely unawares, they'd stop their cars at least a bock away and proceed carefully towards the house/apartment the criminal was hiding in on foot. And sirens. Don’t forget sirens. And why do they always tell someone to "go around and cover the back"? Many homes and businesses have side exits as well. Just once, I'd like to hear: "Dan, you take the side door. Linda, make sure he doesn't exit from the porch. Carl, take Lenny and cover the basement slider. Jessica and Barney, knock on the front door. I'll be in the squad car checking the Lakers score..."
